Same Problem!
In my case, i had the same problem of no menu bar. When i right clicked on the tool bar, it showed that the menu bar was checked. I could not uncheck it either, it remained checked no matter what i did; and still no menu bar.
Due to this post, i right-clicked on the tool bar, clicked 'links', and the 'links' bar appeared, but best of all the 'menu bar' item was now unchecked!
I then right-clicked, clicked 'menu bar' to check it, clicked 'links' to uncheck it, and finally got what i wanted: the menu bar to appear.
This was a lot harder than it should be; it boggles the mind...
